Uso del metilfenidato en pacientes con cáncer avanzado

Resumen

Introducción: el cáncer y su tratamiento se asocian con una serie de síntomas físicos y psicosociales que deben ser identificados y tratados, como fatiga, depresión, somnolencia y delirium hipoactivo para optimizar la calidad de vida de los pacientes. El metilfenidato, una droga psicoestimulante empleada para el tratamiento del trastorno por déficit atencional, se utiliza en el ámbito de la medicina paliativa para el control de estos síntomas en pacientes con cáncer avanzado. 
Objetivos: el objetivo de este artículo es examinar la evidencia que justifica el uso de este fármaco en el manejo sintomático de los pacientes oncológicos, así como su eficacia y seguridad en esta población de pacientes. 
Método: se realizó una búsqueda en las bases de datos Medline, Pubmed y Embase en idioma inglés y español desde el año 1990 a la fecha. 
Conclusiones: el metilfenidato es el psicoestimulante más usado en medicina paliativa. Al aumentar el nivel de alerta cerebral presenta un beneficio potencial en el manejo de síntomas en pacientes con enfermedad oncológica avanzada como la fatiga asociada al cáncer, sedación vinculada al uso de opiáceos, depresión y delirium hipoactivo.
